I hate to sound like an complete idiot. But we just had a class that went over this...our instructor is very "incompetent" when it comes to teaching. She was a nurse who worked in SNFs around the South County (Calif) area. But she turned to teaching after a LOOONG (10 year) vacation from actual nursing. She maintained her license, but she lets US teach the classes. One of the classes that she attempted to teach was the one dealing with NANDA guidelines and Nursing care plans. She's thoroughly confused all of us. Now, I know that there is no magical formula that is going to make the whole "related to/manifested by" issue easy as pie. But is there something out there that can make this process much simpler than what she has mussed up?!? Seriously, is there a "dummies guide to" Nursing Diagnoses or some computer program that can critique format and content? Our class is in dire need of direction at least in this area, because we start our clinical rotation in a week. Plus, we need all the COMPETENT help we can get!!! HELP PLEASE!!

Sparks and Taylor's Nursing Diagnosis Reference Manual is an excellent nursing care plan book that you can use. What it does is has the list of medical diagnoses in the back and suggests which NANDA diagnoses you can utilize. Also, the interventions are excellent, and easy to comprehend. You can order this book from Amazon.com.
I forgot what crappy book my program told us to get, but, when someone suggested this to me, I purchased it immediately, and did not tell a soul that I even had it. I used to do my care plans within a few short minutes...period. I am not suggesting that you don't share with your classmates...that is a personal decision. But, for me, my nursing school experience was really bad with disruptive students who did not wish to do their work. I didn't want to get too involved with them, so, I just did my own thing. Good luck to you! I am almost positive that this book is what you need.

We used the Handbook of Nursing Diagnosis by Lynda Juall Carpenito-Moyet, published by Lippincott Williams & Wilkins. It was very handy to get through all of the careplans we had to write in school.

i write about and answer nanda and care plan questions on the allnurses student forums all the time. here are some threads you will find helpful.

a very nice easy to understand book on how to write care plans is nursing care planning made incredibly easy published by lippincott williams & wilkins. it comes with a cd that has 150 care plans on it that you can edit. the book also has a complete list of the nanda nursing diagnoses as of 2006. it also has the maslow hierarchy of needs. the different types of care plan presentations are also addressed (care maps, critical pathways, traditional, computerized). the 5 steps of the nursing process are pretty extensively covered (the 5 steps = a care plan) especially assessment and documentation. cost is about $40.
