Mood and Behavior tracking

Ok, now I need to see if someone has any suggestions. When our state case mix review was in here, they said that in order to capture payment on Section E, we need to have some sort of tracking or documentation in that 30 day window to show what the mood and behavior issues were.

We already have the CNA's completing behavior tracking due to psychotropics but, they want something more specific to also cover mood. Anything I have looked at is time consuming for the nurses and am trying to prevent giving them additional work to do.

CareTracker is great as far as a reduction in paper and it is really simple to use after you get the hang of it. However, I sometimes find that the CNAs will input info into it and neglect to tell the nurses. If there is a mood or behavior issue, it is supposed to be followed up with a nurses note, which can't be done if we don't know about it. We check for completion compliance near the end of the shift, but that doesn't specifically say what is missing. A minor drawback, and maybe it's just the way my facility has it programmed, is that you can't pull up a specific shift's meal percentage for say supper, or the fluid intake for just dayshift. It only prints out a 24 hour total/percentage.

It must be the way you have it configured. I can pull up all the patients' lunch percentages for one day or one patient for one meal or for 7 days or 14 days or whatever.

Do you do the daily audits? We have our day aides audit the info put in by the night aides, the evening aides check up one the day aides...etc
