Can anyone help with careplans? HELP!!

Help how?
having a hard time with nursing Dx and rationals. My instructor is pretty stricted with me for some reason.
I use Wilkinson (nursing diagnosis handbook) and a couple of others. Wilkinson has a section in the back that suggests diagnoses for different disease processes. I use Maslow for rationales as to how I rank my diagnoses.
I use Doenges (nurses pocket guide), because many of the interventions have rationales. It also has a section that divides the nursing diagnoses by functional health pattern.
Ackley and Ladwig (nursing diagnosis handbook) has an appendix that divides up the nursing diagnoses by Maslow.
Those three books have been invaluable to me in the care planning process. I've bought other care plan books, but those have done the best for me.

i answer a lot of questions about care plans. you should be looking at and reading the posts about care plans on the nursing student forums of allnurses. here are links to get to the two where care plan questions frequently are asked and answered:
there are three threads within these two forums that have a great deal of information to help students writing care plans. you should find some of the information there very helpful. here are links to them:
i can help you with the choosing of nursing diagnoses for your care plans, determining outcomes, writing nursing interventions and rationales. however, you need to ask specific information. to say that you are having trouble with nursing diagnoses and rationales is too broad and general for me to help you. i would have to write a book for you.
you can follow the answers i give to students in reply to questions about their care plans by using the advanced search. search by the word "diagnosis" or "nanda" and specify for posts by me by my screen name "daytonite". this will bring up a list of links to posts i have made on those subjects. those are two words that i use in almost all posts i make in response to care plan questions. i must tell you, however, that you are the first student in months asking for information about rationales. when you have a nursing diagnosis, i will be able to give you specific web sites that have nursing interventions with rationales for them for most of the commonly used nursing diagnoses.
when you are working on a care plan and have questions post them in a thread you start on one of the nursing student forums i've listed above, or you can always pm (private message) me. i always check the nursing student forums for threads needing answers first. i am not on the computer 24 hours a day, however, so please don't wait until the last minute to ask questions and expect an answer from me within minutes or even a few hours. i do have a life beside allnurses!
