hey everybody anybody seen the q15 forms that graph pt behaviors,used to use them in a another hospital and would like to see them used at new facility.can anybody help.

hey thanks for the replies so far .a q 15 is a form that monitors and tracks pt behavior,where they are on the unit.

We use those forms at my facility for q15min checks...they are coded with a letter for behavior, and number for location.

ohhhh....Q15 as in every 15 minutes....I'm having a big "duhhhh" moment here!
