Why don't you do your own care plan? There are several stickies explaining care plans and nursing diagnoses on this site. I'd start there and do your own then post what you come up with and we can help you.

It is perfectly acceptable to ask your clinical instructor for an example of a care plan. Most clinical instructors have different requirements and I always found this to be an important step. I could post/send you dozens of mine but they would be almost useless unless you had the same instructor and were in the same semester as I.
Ask you instructors for a grading rubric as well -- only 1 of mine had one they were willing to share.
You could always join you school's SNA and meet a couple of upper classmen. I gave copies/examples of my care plans to some first semester students who asked. People are funny about sharing care plans for some reason -- I am not.

Hello, My experience of Care Plans has been pretty good thus far! :) This is what I did...
*Every instructor has different expectations so step one talk with them they will be you best bet for help.
*The Main thing I find to keep in mind is ALWAYS keep in mind to make your Care Plans Client/Patient "SPECIFIC".
*Care plan books are also a wonderful investment to get you thinking about ideas. I bought "Nursing Care Plans, Diagnoses, Interventions, and Outcomes" 7th Edition By: Meg Gulanick and Judith L. Myers
I love this book it has inspired me to be creative with care plans, mind you I only use it to reference I never copy out of it...Patient specific is important!!
*My fellow senior nurses have been a tremendous help as well!
Good Luck!!
