Definitely check on the BON and their stand on what you can and can't do, especially if you have nothing at work that covers you. At the end of the day it is your license at risk and if not covered then the facility will not back you up if nothing is in black and white and something happens
As an RN in most areas we aren't allowed to mix up medications like that. (If I am understanding correctly). In fact at my last hospital the pharmacists were only allowed to mix bags of IV meds under a hood and in a special suit.
All out Rocephin comes in pre-made bags where you just break the seal on each portion of the container.

As an LPN in my state, I am not allowed to mix. But I can hang solutions that have been mixed by an RN.
By mixing, I mean, for example, adding 10mg metoprolol to a 25mg minibag of .9%NaCl.
Some abx come in a plastic vial that is attached to the bag of solution, and you just pop the stopper off the vial and mix the solution and powder together in the bag. That is reconstituting, and I am allowed to do that.

Can a LPN mix IV Rocephin in a bag of Dextrose 5%. We were taught that we are prohibited from mixing. Then i read the nursing laws and it said except preparing or reconstituting an antibiotic additive which is permitted.
