How does the 504 process get started?

Curious for a new Type 1 diabetic, how does that process for a 504 get started? If the student moves to a different school, does the 504 follow them to the new school?

The 504 process can be initiated by either the parents or the school and it does follow them from school to school. We hold transition meetings when we get new students who have a 504 or IEP from another school so we are all on the same page. We meet at least yearly to review and tweak the plan and more often if something changes.

Typically the 504 from the student's school is in place when the student transfers. At my school, that student is put on our meeting list to review the 504 (transition meeting) and update it yearly.

As for getting it started - a parent can start the process, a teacher can start the process, anyone can. I have on more than one occasion for a student I know needs one. Not necessarily because I feel my school is not doing what it needs, but because I am also thinking about state testing if student needs stop-the-clock testing or if that 504 could help a student get what they need in college (i.e. a private restroom in a dorm) since some 504 accommodations will be met in college. But I work with the other kids.

Side note:

Most of the students I have that have Type 1 Diabetes have a 504, but I do have one parent that doesn't feel like their child needs one. That parent is a nurse and the teachers have been great and understanding and student is very compliant. I check in about every year, but still doesn't want one. That student also brought me a card for nurse's week :).
