I may have burned a bridge or two, I have alot of varied experience, how do I start over again, I have been through alot of personal stuff and health stuff but I am now physically strong and mentally ready but how do I convince HR of this? any suggestions?

Personally, I would not spend the time or effort applying at a company where I have already been branded 'not eligible for rehire.' When you say that you have burned some bridges, I assume that this is what you mean.
To start all over again, I would establish a solid working relationship with a new company. I wish you the very best of luck!

If you have been out of the workforce for a while you might want to take a refresher course.

You might find yourself in a situation too where performance anxiety becomes an issue. As you said, you've already burned some bridges, so I would assume your options for new employment are getting thinner. Upon finding a new job, you may feel pressured to "make it work no matter what." This is a bit much to carry on your back along with trying to return to nursing.
Don't be too proud to utilize any EAP the company offers. Once you are hired, start with them right away, make a pre-emptive attempt at burrying any problems that may arise.
