quotes are in communication or quotes as in the patient understands when he/she states this. or the nurse should say this.

Are you from a foreign country taking the NCLEX. Is English is your 2nd language? Brushing up on reading comprehension and English before taking on communication questions will help. and will help in general with the nclex.

are you taking the PN or RN nclex.

Oztizzz, general rule of thumb:

Do not ask yes/no questions to a patient (it cuts off communication.) The only exception to this is when someone is suicidal. In this instance, it's okay to say something like "Are you planning to kill yourself?" "Do you have a suicide plan?" This is a safety issue.

Do not ask 'why' questions as it seems judgmental and attacking. Instead, pick a choice that says something like "I notice that you seem worried." Don't pick "Why are you worried?"

Do not give false reassurance. For instance, do not say to the patient, "Don't worry. Everything will be okay." For one, this might not be true. And secondly, it negates the patient's feelings and might cut off communication and expression of feelings.

Do not put the nurse into the statement. For example, do not say something like "I have been through a divorce as well." This may seem therapeutic, but it draws away from the patient's concerns and focuses on the nurse.
