Help with Care Plans

I generally write it as a three part diagnosis....... for example:

Knowledge deficit (or deficient knowledge, same thing) r/t surgical procedure as evidenced by verbalization of questions related to procedure and recovery process.

Knowledge deficit r/t basic infant care as evidenced by difficulty diapering and dressing infant, increased anxiety level during infant's bath and repeated statements, "I just don't know what to do with a baby".

I would do a 3 part too. I only write 2 part diagnoses for risk diagnoses and wellness diagnoses.

Does anyone have a template I can follow for coming up with diagnoses based on my patient's chart and what I collect from the interview? :balloons: :balloons: :balloons:

Ackleys nursing diagnosis book has a list of nursing diagnoses for various disorders. You can then look up those nursing diagnoses in the book and it will give you r/t and AEB/AMB for it as well as goals and interventions (you just have to pick the ones that apply to your specific patient - the interview with the pt and the chart will help you do this)

this is a newer edition than the one I have (mine is orange) but I assume it is the same thing. It works for me!
