    We would like to help but we need some information first. What year are you in school? Why do you need this survey? What is this information going to be used for?

    The first question is flawed. LPN and RN are not degrees, they are types of nursing licenses. I am an RN but my degree is BSN. You should ask "What license do you hold?" and separately "what degree have you earned?"
