Hello.. Need help on assignment X_X

I'm still a student nurse and I had this 1 month old patient. When I was endorsed to give care to him, he seems alright. Since I was restricted to give medicine to the baby, I could only take the baby's vital signs. Then.. We were tasked to make a nursing care plan about our patient and I badly need help X__X I couldn't think of anything for my nursing care plan T__T

what's the baby's diagnosis? since you've taken the baby's VS, did you notice any alteration? temp? RR? apical pulse? is he/she having diffulty breathing?

Nursing care plans are very similar all over the world.

dchen. . .all nursing care plans start with doing a physical assessment and a review of the written information about the patient that is in their chart. without any of that information i cannot even begin to help you. the care plan is based on problems the patient has. those problems are developed from the abnormal symptoms found on physical assessment. if you can give me a list of those, i can help you start work on putting together a care plan.
