For this section you just have to print your last, first name along with your student ID.. then, print, sign and return it. The fsw staff will take care the rest.

I would suggest passing a line and adding the correct location and sending an email along with the attachment letting them know of their error.

Does anyone know if the Hep A is optional to take? I only ask because in castlebranch it does not show you need it, however, it is asked in the health form.

For the MMR and Varicella if you have the 2 vaccines and it is documented you can just upload that. If you don't have the documentation you can provide the titer to show the positive titers. It is either-or. For example, I provided my vaccines for my MMR and a positive titer for varicella because I had the virus from when I was a child.

For Hep B it is highly recommended to get the series of vaccinations and the titer however, if you don't have it, you can sign the Declination Waiver.

I personally did not get all the titers completed because it is pricey for me and only got what I really needed...but it's up to you. So far all my documents are approved in castlebranch. Good luck!

How long did it take you to get results of the titer and what kinda price are we talking?

I’m having the hardest time getting this health form filled out.. my doctors office is unable to help.. the health department does not do titers nor a skin tb test .. I’m in my thirties .. I don’t have records of my previous vaccinations. Any advice would be greatly appreciated. Also, how many series of hep A need to be competed and how far apart are they?

Has anybody in the Charlotte area gotten or ordered their scrubs yet from the store listed? If so, is it easy to have the store order the ones we need and send them to be embroidered if they didn't have the correct size available? I was also wondering if it's cheaper to get the polo shirt and have it embroidered from them compared to buying it from the bookstore. I couldn't find the nursing logo polo on the online book store.

I went to my PCP and she drew my blood on Friday and by Wednesday I got a call to pick up my results (this happened about 3 months ago). I guess it is based on when you go and where you go. So, I would say 2-3 business days.

My insurance covered a percentage as she ran it under my annual physical and I came out of pocket about $170.00 (my insurance sucks). I am pretty sure you can find it cheaper in your nearby clinic or urgent care. You can get your TB skin test done at any pharmacy clinic such as Walgreens/CVS:

TB testing

There are 2 series and will be completed 6 months apart. This is optional so don't stress now about Hep A.

The bookstore on the Lee campus has them in stock and with tax it was only $21
