Frustrated and Need Help!!!

Iam So Frustrated, I Can't Stand It!! I Am An R.n. Of 30 Years And Work In An Snf. We Take Everything And Anything Here, Anything No-one Else Will Take. We Have A Lot Of New Grads. A Situation Arose Last Week That Really Sent Me Into A Frenzy. I Was Charting At The Nurses Station And Overheard A Conversation Between A New Grad And An M.d. The Grad Told The Md This Residents Blood Sugar Was Up Over 400, And Got An Order For 60 U Of Lantus. The Resident Is A Tube Feeder And Getting A Diabetic Feeding, Also Unresponsive, Not Stuffing Her Face With Candy.

I Asked The Grad How Long Her Sugar Was That High, She Replied It Was Going Up For A Week. Looked At The Chart And Told Her She Should Look For Another Reason Why Her Sugars Were Up, I.e. Infection, She Should Culture Trach, Blood Urine And Stool, And To Call The Md Back And Give Him The Whole Story, Since This Is An On-call Doc., He Doesn't Know The Resident. Due To A Emergency On My Hall Iwas Not Able To Follow Up On The Situation Until The Next Day. She Did Not Make The Call Back And Nothing Was Done Until I Came In The Next Day. The Resident Had Such A Severe Uti She Was Sloughing Off Renal Cells. I Tried Again To Explain What Happened To The Grad, She Didn't Want To Hear It,

Became Furious And Stomped Off. I Now Feel Like I Have To Monitor Her Residents Along With Mine, Which Pushes The Ratio Up To 36:1. Help!!!!!!!!!!!!!!!!!!!

May I ask, how did you get each word to be capitalized? Did you do that manually yourself or is that an automatic thing the website did to an all caps post? I was at another website that wouldn't let you post in all caps in the title, but would correct it in this way.

As far as your concern, and back on topic, you sound pretty stressed out. I'd take a deep breath and then take my concerns to your manager since this new nurse wasn't receptive to your concern.

I'm surprised she didn't listen. I love getting constructive advice from those who know more than I.

in the dictionaary are two definitions for new grad nurse:

1.] she who will obey and learn from it

2.] she who knows all the secrets of the universe and will drive her co-workers up the wall.

methinks she is number two.not to put more on the real nurse here, but there needs to be follow up and a write up if #2 will not listen. a ten miniute class might fill in some gaps. #2 may catch on when she has to defend her actions to the nm or even to the bon. does she know that?

yes a hard lesson is a lesson learned hard.

That is a bit scary. Ya, you gotta treat sugars that are going up. BUT, there is a reason for this upward trend and you really have to find it before shoving meds at it that might mask it. Whoa.........The inability/non desire to listen or consider advice from more experianced personell is frightening.

wow..I am a brand new grad myself of only 2 weeks ago......Sounds like the new grad nurse didn't look at the big picture or look at other factors..just the fact that the bg was high, the tunnel vision thing .......

I learn alot from nurses I know that come into my home for my daughter, which is why I can see why you'd be so upset over this.........and understandably so. I know I will never , ever not listen to nurses who have way more experience and knowledge than I do when I start working somewhere( & hopefully very soon)...not a great way to start off a career....

I also don't understand why if this was going on over a week why didn't she question it around then???? Hmmmmmmm. Not rocket science to have an idea something may be going on.........Hopefully you can talk to her & maybe help her understand that she needs to have the "when in doubt, check it out" type of thought.....
