Our was PIE- problem, intervention, evaluation. Same thing, different wording.

Sometimes I get a little nostalgic for the simplicity of old-style charting. Paper charting wasn't perfect, but it took a whole lot less time to fill out and write in an assessment and vitals than it takes to log in and check endless boxes.

In psych, we had BIRP (excuse me!) notes: Behavior, Intervention, Response, Plan.

The psych unit at my hospital still uses that, but "data" is "description".

But wait ...was that all the charting, or just one of many many things?!
