hi could neone please help me with this case study asignment, im having issues finding 5 nursing diagnosis and interventions , appreciate the help thanks
Mrs Chang is a frail and thin 82 -year-old woman in late stage Parkinson’s disease. Until recently she has been living at home with the support of her 86-year-old husband Mr Chang. Mr and Mrs Chang have been married for 65 years and have one child, who died tragically in a Motor Vehicle Accident (MVA) 12 years ago. Many of their friends have pre - deceased them, and as a consequence they have very little social contact with the outside world.
Two weeks ago, whilst mobilising to the bathroom, Mrs Chang tripped and fell becoming trapped between the shower and the bathroom door. In a panicked state, Mr Chang called for an ambulance and Mrs Chang was admitted via the emergency department to the ward you are working in. Diagnostic and radiographic studies confirmed that no bony injuries were sustained as a result of the fall. However, given Mr and Mrs Chang’s social situation and Mrs Chang’s declining cognition and mobility issues the medical staff thought it best to admit Mrs Chang for further assessment and evaluation.
past history , Parkinsons disease, hypertension, Atrial fibrilation, rheumatoid arthritis to both hands
further issues include unepxlained

What do you have so far? Have you looked in your NANDA for nursing diagnosis? I don't mind helping you once you get started, but you need to start first.

i think your text book will provide you with at least 3 nrsg dx's. from what i can see it sticks out pretty well. not trying to give you a hard time but you need to study this on your own so it won't be such a struggle for further assignments. and also be careful posting your homework on this site. plenty of professors are members of allnurses.
thanks for that. there are also issues with polypharmacy
Following her shower, while assisting Mrs Chang back to bed she asks you for her handbag which is in the bedside locker. When reaching into the bedside locker, you notice a large paper-bag full of naturopathic medicines. When you ask Mrs Chang what these medicines are for, she tells you that she has been taking them for years, but is unable to explain why she is taking any of them.
well ive come up with
1. decreased mobility as a result of rheumatoid arthritis
2. inappropriate medication mangement due to deficet of knowledge
3. impaired social interaction due to loss of family member/friends??
4. risk for injury related to altered cognition as result of parkinsons disease
5. self care deficet??
im not sure wat else there is??

I think you have done a wonderful job.
What about: Risk for falls
Impaired transfer ability
Risk for loneliness
Impaired comfort
Imbalanced nutrition: Less than body requirements
Risk for deficient fluid volume
Remember, the fact that she is elderly is a consideration for several of the NANDA dx. There are so many possibilities with this, be very creative.

Culture, customs, possibly language issues? This is a guess due to age of people, the remedies they don't know what they are taking for (really bad English, hope you understand) and names only.
