I have a 2 month baby boy. uncircumcised who went to the doctor with fever of 103.0
test came back with positive for UTI/bacteremia. When he was admitted to the hospital he had no fever. I am confused on what two nursing diagnosis i should do. Any help would be greatly appreciated.
This is also my first time with a peds patient so my objectives and interventions are a bit confusing than medsurg. If anyone has any input on what could guide me that would be great.

why would you be concerned about a 103 fever in a 2 month old?
what are some possible causes of the UTI in such a young child?

You have a very young infant who's ability to fight infection is immature. Why would that fever, and the babies inability to fight infection concern you?
You have an uncircumcised male infant sitting in a diaper with feces...how could the baby get a UTI?
Questions to ask...is this their first baby? Do they have other male children? Are those children circumcised? Is the father circumcised? Do they know how to care for an uncircumcised member?
Now looking at these thoughts what do you come up with?
Care plans are all about the assessment of the patient....you have listed what they have now what did you see? what was your ASSESSMENT of the baby?
