My nursing diagnosis is Risk for falls r/t immobility and uncontrollable movements. I am having a hard time thinking of some MEASURABLE outcomes for a CHILD whom requires total care. Please help!

Risk for falls is a lousy diagnosis in this instance as your related to's sort of suck, but seriously.

A patient who is at risk for falling what DON'T you want them to do? Hell for all Risk for diagnosis this is the case. This not happening is your measureable goal.

I don't know if they "suck" per se. That's not very constructive. Maybe you could be a little clear about your r/t factors.

Risk for falls r/t impaired neuromuscular movement. Just a suggestion. It's hard to help with your diagnoses without having information about "why" this person is total care. Did they have surgery? Do they have neurological, muscular, or skeletal problems? A combination of both. Need more information to help you.

Oops! Patient has cerebral palsy, epilepsy, mental retardation, and mixed development disorder...

Safety is an issue due to seizures. The patient has a self care deficit as well. There is a risk for impaired skin integrity due to injury during seizure activity.

Measurable would be measurable for this patient. Develop a safe environment. How will you assure safety during seizures what could you change?

if you're looking to make the patient safer, how do you know you've succeeded?

like, maybe, s/he (not them, that's a plural...sorry, pet peeve around here) didn't fall?
