all care plans start by you assessing the patient and listing out the abnormal data. it is the abnormal data that become the evidence of the nursing problems for the care plan. the nursing diagnoses are merely the names we give to the nursing problems and you really shouldn't get too hung up on them. if she has cellulitis, where is it? what are the symptoms of this cellulitis. nursing problems are all about how a patient responds to their medical condition and what has happened to them. how did she get a cigarette lighter burn on her forehead in the first place--sounds odd and like it needs to be addressed. see https://allnurses.com/general-nursing-student/help-care-plans-286986.html - help with care plans for information on how to construct a care plan.

ok guys I'm desperate. I have a care plan due next week. My patient had a burn from a cigarette lighter on her forehead. Her admitting diagnosis was cellulitis. A culture of the forehead came back as hemolytic staphylococus. I am really confused where to even start with this one. I was thinking altered tissue perfusion but I'm having a hard time with it. I need the top 2 priority nursing diagnosis and I can't even come up with one! Can somebody please help me? Thank you.
