For pts w/ AROU, how much urine should be cath at max to prevent shock?

What is AROU? Sorry, but brain is not working today if I should know this.

Eons ago nurses were taught to clamp a cath at 1liter. Evidence does not support this theory. The bladder is a reservoir and you are simply emptying the holding tank. The body has already acted as a filter and sent its waste product to the the holding tank. Emptying the bladder does not suddenly cause the body to go into an immediate hyperdrive situation and dump out more urine that would cause shock. Hypotension and shock does not occur because you emptied the bladder.

In a handful of cases over many years and hundreds if not thousands of patients, I have seen bladder spasms occur with pt's who had overly distended bladders and the release of the several liters of urine caused temporary discomfort. This was easily rectified with diverting the patients attention and when all else fails, a B&O suppository.

Check out some of the latest nursing literature if you need to verify this in order to change to evidence based practice. I was pleased to see that this very subject was in the latest Nursing08 journal as this myth keep being perpetuated.
