APN Careplan

Any advice for writing care plans... Having a little trouble in my Advanced Nursing Practice class..

Featured Replies

Start thinking out of the nursing model and consider what tests need to be ordered, what drugs should be ordered, what referrals should be made.

When you say a care plan what do you mean? I wrote soap notes but never an actual care plan.

As PP said just think things through. Think of differentials. Think of how it affects the patient and their family. Do they need home health? A walker? Nebulizer? Do they need to change diet? Can they take their meds appropriately? Have family support?

These are some things sometimes missed by my MD colleagues. I worked home health for a few years before graduation so patient safety, education, and support is a huge part of what I try to do.
