I am doing a paper for class regarding policies we have implemented at work. We obviously have to cite sources of evidence based practice.
I need to use statistics on how the changes have improved our practices, but obviously, these are only at our location and have not been published.
So my question is...how do I cite the statistics of how our facility improved?
My information is from posters that they have displayed on our unit.

Where did the information on the posters come from? I would track that down. Perhaps you can get a quote from someone who collected the data, or cite internally published documents.

If nothing else something along the lines of a verbal interview with the infection control honcho regarding infection control stats for the facility, as an example, done on such and such a date. I seem to remember examples of how to cite interviews. Make certain you get the person's title correct.

Sometimes, charts and graphs will have a reference in tiny print just under them. You can use that reference to build your citation.
If all else fails, you can do the standard APA format and plug in specific information.
Author (year). Title of source. City, ST: publisher.
author = the name of your facility
year = the year the poster was published. You can sometimes find this in the document number assigned to the poster for printing and reordering purposes. Look in the bottom corners or on the back of the poster.
title of source = the big headline across the top of the poster
city, ST = the location of the poster
publisher = "Author" (when the author and publisher are the same entity)
So, your reference list citation could look like this:
General Hospital (2010). Results of infection control policies at General Hospital. Baltimore, MD: Author.
Your in-text citation would be:
blah, blah, blah (General Hospital, 2010).
