Acute Pain problem statement

Having trouble with my problem statement. Its feeling a bit wonky to me!!

Acute Pain r/t physical secondary to abdominal pain AMB current pain level 8-9/10, Tolerable pain level 8/10, PT states: "My stomach hurts so much"

the r/t seems weird to me...tweaks/suggestions?

Instead of using an r/t can't you use a different s/t (i.e. secondary to surgical incision or s/t abdominal injury)? That might be why the r/t seems weird. I don't really understand "r/t physical" and "abdominal pain" is not an s/t without an actual explanation. Is there an unknown etiology for the abdominal pain? If so, then:

Acute Pain s/t abdominal pain of unknown etiology AEB pt verbalizing "my stomach hurts so much" and rating pain an 8-9/10.

To me, the r/t should be abdominal pain. Also, you've confused me on the pain level. From what you wrote, the pt states pain is 8/10, but that his/her tolerable pain level is an 8??? Did you mean pt feels 8 is intolerable?
