I found out today that I got a verbal warning because I didn’t know I had to report a skin tear. I only thought that they were falls. My work agreed that it was stupid but they only gave me a verbal warning because the state got involved. Will this end up going on my actual LPN license because the state got involved? Currently it’s not on there but I’m so scared ?

If the resident did not have a dangerous change of condition you will most likely be fine. You didn't say how long you have been in practice but experience will be your guide. Skin/wound issues have their own heirachy in reporting due to the fact that they are generally caused by inaction of staff. One thing is certain. You won't make this mistake again and I doubt very much this will go on your license. Does your licensesing agency have apublic discipline page? If so take a look at what kind of actions effect one's llicense.

I'm not sure about the discipline page. However, my facility told me they will not take any action and will more so just brush off the whole thing. They told me they only took action because the state got involved. Otherwise, it would have just been a "Hey, do this next time".
I think I'm past my "manic" point now LOL. I feel confident that this won't go any further, and it was just a learning experience!
