Here is a practice question from our study guide for my LAST FINAL!!!!!!!
What do you think the answer is?
A nurse is assessing an elderly postoperative client who is exhibiting signs of delirium. The nurse observes that the client is convinced that it is 1954 and is complaining about "the bugs in this hotel". The nurse's priority intervention should be to:
a. obtain a prn order for haloperidol Haldol
b. transfer the client to a room near the nurse's station
c. call the client's family to come and stay with the client
d. arrange for a patient care assistant to stay with the client.

Do they not give answers to the questions ??
Can you believe that in many of my courses they do not give the answers. Even if you email the instructor and ask, many times they do not get back to you. That is why I thought I would get a concensus from some good minds here.
Well, what do you think the answer is since it's your final ? What was your first gut response?
On the floor I work on as an aide, we would assess the LOC and either put the patient in a room near the nurses' station for slight change, or get a sitter for them for more severe cases. I would actually get cultures on them as this could be a sign of some infection - but that is not an option. I would say D.

I would pick D also because the pt needs someone to keep an eye on him now for his safety. The other answers require various waiting times.

I'd guess D, because the ABCs are all taken care of, and the need for safety comes next, no?

so that nurse can keep observe patient more closely
since the patient is postoperative i would feel more comfortable observing these symptoms myself. when patient has been moved to the nearby room and i become thats only when i would delegate the observation to a cna.
