I was given a verbal warning today @ my job over something I don't agree with.. I work for a surgeon & we have ICCharts to which patient information is put in.. I received a message about scheduling appointment for patient and put in xray from a coworker when I was at offsite clinic... I forwarded the note back to the cma with the message.... appointment has been made & xray has been ordered.... This should have been done while patient was on the phone.... Does that mean that I violated anything concerning patient rights... Please let me know if you could on this please anyone....

What exactly was the infraction for which you were given a verbal warning? Did they state it was a violation of patient rights? Or did they state you failed to follow internal policies?
I was told it was given because it was a threat or something to that effect...They also stated that if a lawyer requested records that because of this statement I put could cause trouble...I have no idea what they are talking about....

That last part looks bad on the clinic. The statement of taking care of it while on the phone should have been left out of the chart

Yes, that should have been addressed in a separate email/phone call.
There are classes on charting offered through some state boards and regulation boards. The classes are very helpful as to what not to chart.
