I do not understand your post.

i am having a hard time understanding your post.. could you clarify what you are saying? i am a RN , but i don't know what you mean by dr's having the right to tell RN's the dosages? dr's have the right to tell anyone who is taking the med, or giving the med the dosages , what is going on... i am not sure what a AIN is.. that is not a nurse a nurse is a RN or LPN.. are you in the US? is a AIN a nurse in another country? a nursing assistant in the US is not a nurse ...

I agree with the others, not quite sure what you're after here. Are you passing meds right now? If you're trying to understand what the directions are, the doctor wrote: "ointment to left knee at night".

As others stated, your post is not clear, but I think the order is to apply ointment (ung) to left knee at night. Somewhere in my brain I recall "ung" being a term for ointment.
