Shot Records...HELP!

Hello everyone,

I was recently accepted into an LPN/LVN program locally. My acceptance is pending my drug test, background check, etc. I don't think I will have any issues with that. However, they do want a copy of my shot records. My mother passed away when I was 18 (before I even finished high school), and I'm not sure she ever even kept a copy of these. Where would I go about getting a copy of my shot records? I don't have a primary care doctor. The elementary/middle/high school's that I have attended are no longer in operation (although the board of education is). My pediatrician closed her practice and passed away years ago as well. Any suggestions? I'm going to call the Health Dept tomorrow to see if they can help.

If you are unable to obtain your records you can have titers done. I am sure the department of health can assist you with finding a location to get the titers drawn.

I also had to have titers drawn because I did not have my shot records. Ask your specific school where to get them drawn at. I was sent to a hospital that provided the test for $36 to students that attended my specific school.

Try your local health unit. When I first started my program I had to get shot records and my mom didn't have copies. I didn't have a local primary care doctor cuz I grew up in another city. I had to drive out of town to get them from the health unit there. All of my found records were then put in the system so I could receive them from any state ran health unit within the state. Thanks everyone for your replies. I had titers drawn at my local health department. My insurance covered everything. Anxiously awaiting results. Baaaah. I hope they come today
