Shift report sheet

I was recently hired onto a med/surg floor and I'm currently going through orientation. Being a new grad, I feel completely overwhelmed and lost so I would like to prepare myself as much as possible. Does anyone have a shift report sheet that they would like to share with me? I have looked at many and still trying to find one that works for me. I probably just need to sit down and look at what I have and figure them out. I would appreciate any advice that you could give.

Is the institution on an electronic medical record? If so there should be an SBAR area. Your institution must have a report sheet that is universal throughout the hospital. What is your preceptor using? Keep me posted….maybe I can send you something.

They are on EPIC and have 2 different sheets that they use for shift report but I used one that I really liked during my clinical. I cannot find that format so I guess I am searching again to see what works for me. Maybe I just need to get into the swing of everything. Very overwhelmed right now with orientation and ECG classes.

We use EPIC too. The SBAR tab, provided it is used appropriately is very good. Are you looking for something for you to take your own notes on
