shared case management

I am interested in finding someone who job shares during the week with someone else and manages a case load for hospice home visits. Thanks Basil

During periods of high census we have had to assign a case load to our part-time nurses. The regular part-time becomes the case manager then and visits at least once a week and other visits are done by prn staff. They must communicate well with each other and share information and discuss the care planning but ultimately the one who is assigned as case manager holds the responsibility.

I only work part-time but have my own pts. If they need a nurse on the days I have off then another nurse goes. Therefore I do all the case management. If something is going on with my pt. the other nurse will either call me at home with an update or update me at Staff meetings on Fri. if it is something that can wait. Often times if I'm am too busy with seeing pts. and do not have time to do the 60day recerts then the other nurse will review the chart and do it for me. But I still will sign off after I have reveiwed it. Hope this helps.

Thanks. My supervisor has told me that on my days off they are really not supposed to call me at all unless I am getting oncall pay (which they will not do). Do you feel like you still have to instantly available for questions on those days or will they go thru someone else if they cannot get a hold of you?? Also does just one nurse cover you all the time or does it vary? Thanks Basil.

Hi Basil! Good question. Your supervisor is right, your coworkers should not be calling you at home. After being called on sick days, vacation days, etc. I began billing for time. Your collegues should be going through your clinical coordinator for questions. We have had major discussion at my place of work re: above. Home health nurses should be able to work independently, problem solve and have good critical thinking skills---when you're off work, they become the case manager for the day and should be identifying and following through with problems. We do have case managers who job share, they constantly communicate with each other. Good luck. I can tell you really care about your patients.
