who is responsible

who is responsible :physician orders medication, nurse gives medication, then checks chart, pt is allergic to drug,,, nurse? physician? pharmacist?

I still say that ALL are responsible. The ultimate responsibility falls on the nurse though. She is the one who gave the meds. The nurse is the last line of defense for the patient.

If you give a medication to a patient who has a documented allergy to that medication, you are the one responsible. No sense looking for the allergy information after the fact.
