Report Sheet

Does anyone have a computerized report sheet they can email to me? I was just given an extra role at work to assist the new orientees with their organizational skills because my boss thinks I do well at this. I know, however, that everyone works differently. I have a couple different report sheets that I have personally used, but am looking for many types of examples I can show to the new grads to help them find what works for them.
