I helped out on the skilled unit this week, and because the nurse I was helping out refused to give me report on the pts I was picking up (she got report and the other nurses left before I got sent over there), I had to go digging in charts to find out what was up w/ my pts. On several of them, I saw the dx HLP, but I couldn't find anything in the chart that could help me figure out what that meant. Any ideas?

....and what do you mean, she WOULDNT give you report???????? where was the sup?

Refused to give you report??? Then I think I would have refused the assignment. Did you go to the supervisor? I would have. That is so unsafe.
I work in a rehab/nursing home. We are supposed to staff 2 nurses for each unit at nights, but when we only have three, the side w/ 2 nurses helps out the first four hours on the rehab side, then the last four hours on the nursing home side. She was not wanting my help for the first four hours of the shift, she wanted my help for the last four so that I could help her pass meds on less than 27 pts (census was 27, but not sure exactly how many ppl she had to pass meds on). When I told her that I was there to help for the beginning of the shift, she kept saying "Well, I don't have time to give you report on these people, because I have to get my stuff ready for in the morning seeing as how you won't help me later." Her getting her stuff ready for the morning included pre-pouring all the am meds at 2400, then sitting down in the day room watching tv. We do not have a sup in house at night during the week, and we both tried calling the on call supervisor (who returned the call at 0630). Then she had the nerve to say "I don't see how you're going to be charting on people you don't know when you didn't even get report." I lost it by that point, told her that if she quit running her mouth and complaining and give me report, I would be able to chart effectively. She shut up and didn't talk to me. So, I checked the dx on my pts and the last two days of charting to get a feel for what was going on.
*apologize for the rambling, worked last night, this is my one day off, haven't been to sleep yet, so that I might enjoy a night snuggled next to hubby :)
