The field nurses are called case managers. They are called that because a patient may have mutiple disciplines (nurse, physical therapist, occupational therapist, social worker, home health aide) seeing the patient and its the nursing responsibility to oversee them all and make sure that are acting in the best interest of the patient. For example, if the nurse notices that a patient is more lethargic today, she may call the physical therapist and suggest that she reschedule the visit.

It depends on the agency. I have been the field RN/case manager and also just worked in the office as a case manager. Some companies even vary based on the office. Currently work for company that has separate offices for private duty, rehab, visits, and Medicare...each one has a different use of RNs.

I am an RN Case Manager who works in the office and sees patients in the field. I do not see all of the patients I case manage though. I have some nurses who are exclusively field nurses.
