  1. I realize that this"Birth Plan"May sound cut and dried but is no more ridiculous than the ones I have read printed off the internet. Things like reaching into the incision and touching the baby during a c-sect and other things. I have worked L&D for many years and have seen more people show up for natural delivery with no more preparation than for a pedicure. The "Baby Story' is not real. Babies rarely come in 1/2 hr. Labor hurts and your doula is not legally responsible for your care. We don't have time to let you decide if your baby is crashing and you need a c-section. Dont gain 85 lbs and expect an easy time. Don't be induced if you don't want to be monitored I will bend over backwords to assist my pts. but prepare yourself for no pain meds. And most of all don't let your SO, sister mother or any other person speak for you I respect this DR for letting his pts know in advance
  2. Be very careful and think very long and hard about Alaska. They will court you and promise you paradise but travel out of the state is very expensive and dependent on the weather so if you want to get home you may not be able. These companies will move you up but you are on your own to get back As far as the charming people in the Bush,if you are at a native hospital expect verbal abuse and lack of manners worse than any inner city hospital in the lower 48. My advise is travel there 1st so you have escape funds available and talk to nurses not recruiters where you are thinking of going. One more thing is there is a Native preference for hiring and while that may not effect an RN it will make things tough on a spouse seeking employment We are out of here soon M
  3. Is Kodiak as pretty as i have heard. Barrow is not exactlyscenic but there is wildlife aplenty and all the snow you could ever want. We have a 5 year plan here but really want to visit other parts of the state,I am still in a bit of culture shock at the hospital but work with a pretty good group.
  4. I have been working In Barrow AK (furthest north point) since Dec and was wondering if there are any others out there with similar experiences. I am a permanant nurse, not a traveler,that left a large metro area in Fla for the snowy tundra.We are a small hospital in a town of about 4000. We see all kinds of cases and get a lot of medevac stabilization.My previous focus was high risk(high tech) OB and am currently working all areas. Just thought i wouldcheck to see how others are doing.

OKAY "My cousin had her baby at 34 weeks at it is just fine"(Yes the intellegence level in your family is phenomenal) "Is my Doctor here"(yes they sit here all night just waiting for primips to come in for very early labor checks) "I dont like the way my legs feel" (Ok we will turn off the epidural and let you feel the contractions instead") This is just a few but there is a lot more.

Has anyone out there attempted to start a 18 gauge IV on a mom with 60/40 BP after a huge bleed.Has anyone had a Mom beg you for pain meds or epidural only to have a FOB or coach tell you no that she did not want them. I support every womans right to choose her birth plan but I know that many are filled out on the internet with out knowledge of the rationale for these interventions. I am one of the few RNs on my unit who will volunteer for natural pts but I am also there for my pt as her advocate not her coach or any one else. I do not believe that an INT is going to interfere with her labor.I am a professional not a maid and I think after some people practise a while the will resent that implication as much as the rest of us
  7. I cant help with the nutrition question but have felt a 10 cm cervix held open with a bulging bag go back to 7 cm when the amniotomy was performed. Of course the pt was draped and nursery and all were in the room but waht can you do. The possible alternative was a nurse delivery if I had not called the Doc and that is why they get the big bucks....

    We keep our standards to 30 min "decision to incision" for most stat sections we always have an ORT present and our own OR. We never use the main OR. Our holdup may be no OB in house or anesthesia. We have a stat pak ready to be opened and used . If no count we get x ray before the pt is extubated or final incision closes

  10. Check into utilization review as an entryway. Dont limit yourself to hospitals look into insurance companies/HMOs. Another big field that is opening up is disease management . I have done them all but returned to the hospital because I cant stand sitting down so long. When tou buff your resume stress care coordination and any d/c planning you helped with. Good luck.
  11. I guess I could site the example of the (newbie) CNM I worked with one weekend who insisted on doing her own vag exams and ended up with a breech baby with no MD back up in the house. Any experienced L&D RN could have assessed that breech way before the pt was complete. There is something to be said for those years of practice and criticle thinking skills. This CNM went stright to midwife school w/o any L&D clincal experience.

  13. Congratulations !!!!!I used nicorette gum because it gives you something to reach for and an instant response. Unfortunately I used it for 2 years but hey its better than smoking and now I am nicotine free. Drink lots of water and exercise helps too. Hang in there. I lost my brother to CA this summer and it stinks to loose someone you love way too young
  14. I am RN with 15 years exp in many varied fields of nursing(thanks to the Navy)We want to move out of Florida and away from immediate threat of hurricanes. My dh retires from the navy in May 2006 and we are looking to go north. I want a milder summer, and cooler weather. My recent experience is in L&D and Day surgery. We are also looking to get out of the social rat race of the "burbs" and start over. Any recommendations?:)
  15. Are there any other areas that might not be so pricey but will have at least a level 2 L&D. It is nice down here but soooo hot and humid in the summer and way too many hurricaine. I am sick of the knotted stomach and fear that we will lose all.
