Asked to Write a Formal Explanation
I am an RN in CO at an Addiction RTC & today, while on break I layed down in the station to rest. I fell asleep during that & allegedly a CNA tried to awaken me once & I didnt wake up. After 20min of napping I woke up myself & during that nap the CCA called another RN at a sister facility claiming she was concerned of impairment, as I am not able to afford my heart meds (due to a company email hack where I spent 1000$ on gift cards when CEO email was hacked.) I passed my UA & Breathalyzer as well as neuros. Now I have to write a statement. Needing any help, suggestions, etc on this. Msg from Doctor: Hope you’re feeling OK. With the concern by the cca will need to go through proper protocols. I’ll just need you to write up a formal explanation. I’ll see if I can come in during your shift Sunday or Monday to meet.

Licensure endorsement compact state
Currently I have a active unencumbered RN license multi state compact in the state of Texas I am seeking to relocate to Colorado for a Nother are in job I went online to NUR SYS.com And completed the licensure endorsement verification and paid the fee to have my license verification sent to the Colorado board of nursing... is that all I need to do since they’re both compact states because I’m reading some parts online where you have to fill out a whole separate application and do a whole Nother background check in the state you’re transferring to I’m just so confused

TX BON Letter
If any one would like , or also comment if you this would help others, I can submit the report and/or Declatory Order made by my attorney as maybe a guide for any other nurses out there interested in this or needing help themselves?? Or would this be a bad idea ?? (I would obvi black out all identifying info )

Thank you.. I wish there was more. My attorney told me it was due to the security officer also being a sworn peace officer he had a duty to report. Due to that officers dedication to board rules or however you want to view it, led to him submitting the police report

Whoops forgot to add I then OD'd following that event and that's why I was in ICU. It was a stupid mistake but I wasn't thinking right then.

I found my roommate & best friend dead from intentional OD from meds she obtained from my room... I was in a dark place then. MUCH Better now through the help of counseling

Update I got a letter today ironically and now they’re wanting a written description and my words of the events leading up to it what happened afterwords and medical records and I don’t remember any of it so this is going to be hard and I would think a police report would be good enough for them. I have graduated with my BSN but they won’t even let me take the NCLEX-RN yet

Yes my attorney said because it was a certified peace officer he had to present it to the court and the court decided on filing a warrant for the property damage even though the ventilator is insured by the hospital. And according to the medical records I was coming off fentanyl, versed, and Midazolam (although that was admin 1hr prior to the “event”)

I was in an intensive care unit and after being extubated I apparently got violent and knocked over ventilator and security was called and the security officer just happened to be a certified piece officer needless to say three months later I get a felony warrant for state jail felony criminal mischief property damage at $5000 . I got a letter from the board of nursing after I sent them my declaratory order from my attorney that was back in February . I received another letter saying I needed to pay $150 for the investigation in the enforcement dept. My question is what stipulations what I even face with this not being a drug related or violent crime and how long does this investigation usually take and what does it entail any help ❤️
