  1. I am graduating with my BSN at the end of the month and would love to get some information on the MSN program through WGU.

  3. An incident report should be filled out. Document just the facts with no emotion. It wouldn't hurt to make an appointment with the CNO and make sure that she is aware of your side of the situation. The other nurses wanting the physician to get in trouble sounds kind of childish. In my experience, it is not very often that a provider will actually get in trouble, more than likely it will just go into some kind of peer review. The longer you stay silent the more it may look bad. Honesty is always the best policy.
  4. Thanks everyone. It is my personal practice to document the time they leave, how they were transported and when they returned. I am looking for literature or best practice so that I can share it with those that don't document times.
  5. I am trying to find information on the documentation of times for when a patient is off the unit (ex. going to radiology). I document the time a patient leaves and then the time they return but others say that the nurse should not be responsible and that the ancillary department should do it. Does anyone have any information on this? Thank You!

  11. I am looking for a totally online RN to MSN program with a specialty in Nursing Informatics. I have narrowed it down to Excelsior versus Chamberlain, and have read a lot of reviews good and bad for both schools. I am hoping that some of you could help with my final decision. Does anyone have recommendations either way? I am not looking for easy, I am a hard worker and went to nursing school while raising 7 children. I work 40+ hours a week, and do a lot of volunteer work. I just need a program that will be flexible. How is the course load and can anyone give me an example of assignments? A lot of online colleges are out because I live in NC. Thanks in advance.

  13. I am trying to think of a nursing diagnosis to use for keeping the baby safe during the hospital stay. I have the goal of "Infant will remain safe during hospital stay" and I have 6 interventions: 1. Match ID bands, 2. Keep unit locked, 3. Transport infant in bassinet etc.. Been racking my brain and using google for a few weeks and cannot figure out what to use for a diagnosis. Any ideas?
