Feeling guilt, please respond
I work in a sub-acute rehab. Last week we had a patient who is admitted with pancreatitis. I received him four hours after his admission from the hospital. Immediately upon meeting him, he wanted to leave against medical advice. When it was explained that he would not receive services at home if he left AMA, he decided to stay He then complained of pain to the lower left back where he had a surgical incision, and I gave him Tylenol for the pain. He was prescribed Percocet but this was only 5 hours after admission and we did not yet have the script from our doctor approving of the narcotic. He then started complaining of other abnormalities. he complained of generalized pain for which I called the doctor. the doctor called in his script for Percocet, and that was it. however, at this point all of his vital signs were within normal limits except for a slightly elevated pulse. For the post I had him perform a valsalva maneuver. This seemed to help with some relief. He then started complaining of just not feeling right. again I took his vital signs everything was normal, I auscultated his heart over the aorta with S1 s2 no murmurs. Just tachy. Apical pulse 100. Lung sounds clear, no SOB breathing easy and unlabored. No JVD, cap refill brisk, normoactive bowel sounds. Skin turgor good, skin clear and pink. and I called the director of nursing to come assess him. she did and said that everything was normal. I called the doctor about his complaints and she did not have any new orders. He then complained of shortness of breath I called the nursing supervisor who called the doctor and got an order for duoneb nebulizer treatment and to send him to the hospital if the shortness of breath continued. We apply the nebulizer and within 60 Seconds he started exporating white sputum and went unconscious. He had no vital signs. CPR was initiated. 911 was called, a code was called. They weren't able to bring him back. I know this post is very scattered but my brain is very scattered right now. I feel like I killed this man. That my lack or knowledge or experience prevented him from the proper care.

What ACTUALLY works in nursing school
Recording the lectures and listening quite a few times while taking notes from those recordings works best for me. The semester I did best on, this was my study method. I just graduated my RN program last week! :)

New nurse, just want to walk out
I felt the same way when I first started. I only had five days of orientation. I worked 7-3, but often left hours later to complete my work. I was so overwhelmed and stressed. I felt like quitting everyday. I had dreams about making mistakes at work, when a patient died (on my weekend off) I blamed myself and thought of things that I may have overlooked that could have helped her.. I completely empathize with your anxiety. It is very difficult. For the first month, I came in an hour early to search through the MAR and TAR for things that I had to complete during my shift, and the time frames where these things needed to be done, who had appointments, etc. I work on a subacute unit, so my patients were revolving very often and were often post cardiac/orthopedic surgery. It gets better. It really does. After some time, you will feel like maybe you do know some things, and that you are a decent nurse. Right now you may feel like you are drowning, but you will succeed. Everyday you will learn new things to better serve your patients! Keep learning, look up medications and diagnoses you don't know when you are at home, and keep pushing!

LPN to Rn in northern schools Nj
I'm not sure. I know you can opt out of the entrance test with certain SAT scores. I was able to work full time the first semester (second semester of the program since we skip fundamentals as LPNs) but this semester I cannot. I just put in my request for part time.

LTC Nurses please respond
1. What size is your LTC facility? I.E. Maximum capacity for residents. 180 patients 2. What is the nursing to patient ratio? On days and evenings, one nurse to 26-33 patients. On nights, one nurse to 50-60 patients. 3. What is the CNA to patient ratio? Usually 1-10, sometimes 1-15. 4. Do you feel you are able to provide quality nursing care to your residents? No, not at all. But we do the best we can. 5. Is your facility doing computer charting? Mostly. MARS and TARS still aren't computerized. 6. Are you subject to mandated overtime? No 7. How long have you been nursing in the LTC setting? Almost one year. 8. Do you feel that you are able to keep your various residents safe? I try to, but no, not always. 9. Does your LTC home have a mixture of residents? I.E. Rehab to home, typical geriatric, psych, homeless otherwise? On my floor, there are plenty of sub acute, psych, and homeless patients. 10. What is your hourly wage (ballpark) and do you feel you are underpaid for the work you are doing? $25/hour, and sometimes yes, given that I am given the same ratio on subacute as nurses in my facility receive in long term care, and no wage differences. 11. What state are you practicing in? NJ

quit my job?
I have been an LPN at a subacute rehab for 7 months now. The job is very stressful, and I often stay numerous hours past my shift to complete my work (work is often assigned to me AFTER my shift ends, like discharges or admissions they want me to help with...) I am also an RN student. I am off school now for the summer, so it is manageable. but the past semester was so difficult on me. I had been working full time, along with full time school. Part time positions are not available at my job. I have been saving money, and have managed to save enough to get me by for the next twelve months (when I finish my RN) without working. however, the prospect of quitting my job is very stressful to me, as I know jobs are tough to come by. I would be able to survive without a job due to me being an obsessive saver and very frugal person, but just the idea is so frightening. I however, do not know if I will be able to survive the RN program with this job, as the time required and the stress levels overwhelm me. What would you do in my shoes? Will quitting this job with less than a year in reflect poorly upon me when applying for RN positions?

experience as LPN help you as aRN?
hello, I was wondering if anyone has noticed that their previous LPN experience has helped them land a job when they became an RN. I am an LPN, halfway through my RN program, and am anxious about this tough job market. By the time I graduate, I will have nearly two years of LPN experience,

LPN job vs RN school
they did not grant it. I gave them my schedule of days off, and still found myself on the schedule. When I inquired, they said it was not approved because the state is doing their survey. This is not the first time they have not followed the days off I have requested for school. They are not supportive at all of my class schedule, and told me that the job should be a priority over school. I know I am going to have to leave because of this.

LPN job vs RN school
hello, I am an LPN working in a LTC/sub acute facility. I am also in RN school. The state just arrived at my facility, and I was told I cannot have the day off I requested (tomorrow, Monday) I needed that day off because I have a test in my RN program. I do not know what to do. I need to do well on this test, but I also need this job. Should I just call out and accept the repercussions, or go in and accept not doing as well as I would like on this test, especially with the stress of the state inspection?

Not paid for working past your shift?
we do punch in and out, and those hours are recorded. However, on the sheet we are made to sign to receive our check, it states the hours we were there, and then it states a "paid hours" section for the hours we were scheduled for (minus the 30 minute break that we all work through but are not paid for). So if I worked from 7-6:30, but scheduled from 7-3, I am only paid for 7.5 hours. Also, we are not allowed to keep these sheets but must sign it in order to have access to our checks...

Not paid for working past your shift?
Hello, So I just got a new job at a nursing home, and discovered that all time past our shifts end is unpaid. Is this normal? Like today for instance. I was ready to leave at 3, but a patient fell and the 3-11 nurse had not yet arrived. Therefore, I was forced to stay past my shift and do incident reporting and charting. and the 3-11 nurse is often late, so endorsing and doing report often happens at 330, when she arrives. This is so unfair to me. Is this the norm?
