Nursing care plan- Health Promotion
Patients vitals- 119/60 hr-74 o2 stat- 93 on room air, rr-18 at resting, temp 36.7 Crackles in lower left lobe Nonpitting edema bilaterally in lower extremities and unilateral left upper extremities Pain- 5/10 radiating from posterior chest to anterior (left side near lung)- comes and goes with movement -patient states "throbbing, and it feels like someone is nagging me all the time." Aox3 cough- non productive but causes sob (has asthma as well) Braden- At risk for pressure ulcers- kyphosis keep an eye 16 Hendricks- High risk for fall 6 Katz- Patient is independent 5 (needs help getting up) Uses cane to steady self when ambulating; good ROM in upper and lower extremities hx of pneumonia- has the influenza shot but not pneumococcal Sorry running to class. If I have time today Ill write more but thanks for all the help guys!

Risk for falls r/t impaired physical mobility due to pain and peripheral neuropathy. Readiness for enhanced nutrition r/t a desire to improve wellness evidenced by expresses willingness to enhance nutrition. or Readiness for enhanced self health management r/t a desire to improve pulmonary function evidenced by desire to manage the illness

Patient is 78 History- COPD, HTN, Osteoporosis, Peripheral neuropathy, GERD. Came in with pain in lower left thoracic cavity- thought fx ribs but nothing on xray. Pneumonia diagnosed 2 days after being admitted.

Hi thank you for your response, My care plan has 3 parts. For the actual- Infection rt secretions in lower left lobe evidenced by crackles. Risk- risk for falls rt impaired physical mobility evidenced by pain in lower left thoracic cavity and peripheral neuropathy. Health Promotion- Readiness for enhanced nutrition rt desire to improve evidenced by low calorie intake but expresses willingness to enhance nutrition. This is what I had at first but my patient did mention stuff about the pneumococcal vaccine. Especially since she has a history of pneumonia. But I was having difficulty figuring out what to write/goals/interventions.

Hi all, I am currently working on a nursing care plan and am stuck unfortunately. My patient has COPD with pneumonia. So I wanted to do readiness to enhanced immunization status r/t desire to improve health evidenced. I am having trouble coming up with interventions. short goal- Patient express knowledge on the importance of vaccines esp pneumococcal. long goal- Patient receive recommended immunizations yearly... Interventions- Short: Assess patient's perception of the pneumococcal immunization. Educate the patient on risk factors of receiving the pneumococcal vaccination. Discuss the risk factors that increase the likelihood of developing pneumonia. Long: Having difficulty with... Any suggestions? Thanks!!

Hey Guys! I just wanted to see if I am on the right track for a nursing care plan also I don't know about the long term goal (if its any good)... Thanks for your help! K Actual: Impaired skin integrity related to malnutrition and physical immobilization evidenced by stage 1 pressure ulcer on sacral area. Short term goals: -Patient will maintain skin intact with no signs of further breakdown by change of shifts. Interventions: -Frequently repositioning the patient every 2 hours or as patient requests. Rationale: "Reduces the duration and intensity of pressure." (Perry, Potter, Elkin, 2012) -Assess the patient's nutrition status Rationale: "Deficiencies in any nutrient results in impaired or delayed healing." (Potter & Perry, 2013, p. 1184) -Monitor patient's incontinence status and minimize exposure of moisture to area. Rationale: "...exposure to gastric and pancreatic drainage has the highest risk for skin breakdown. " (Potter & Perry, 2013, p. 1189) Evaluation: Hypothetically, Long term goals: -Patient will verbalize the importance of good skin integrity by discharge. Interventions: -Educate patient on the importance of nutrition and eating a well balanced diet. Rationale: "A balanced intake of various nutrients (i.e., protein, fat, carbohydrates, vitamins, and minerals) is critical to support wound healing." (PP 2013, p.1186) -Educate patient on the importance of hydration. Rationale: "dehydration can affect albumin levels and tissue integrity." (PPE, 2012 p. 613) -Patient will tell me in detail what is the cause of breakdown in skin. Rationale: Evaluation: Hypothetically, I would teach the patient the interventions needed to protect against further skin breakdown and promote good skin integrity.

I called around March and they said everything was in for me and I sent everything in around the end of April. I didn't get notification yet either so we shall see. Need help with resume~ stressed out, please help
The BLS/CPR is a definite and will look good on your resume. I also volunteered at 2 hospitals a year before I applied to accelerated programs. Also if you do not have a strong resume, make sure you have a strong personal statement. Goodluck!

Not at all, I am a senior at Florida State University receiving a bachelors in Family and Child Sciences. My gpa is 3.5 and pre-req gpa is 3.7-3.8. Thank you and keep your head up! Maybe call and ask an admissions person what you could do better to improve your chances?

FSU Fall 2013 Applicants
Yeah the reg BSN is different. I just came from CON and said the accelerated letters were mailed out today. I didn't get in so I am happy I have backup schools. Congrats Nolesnurse!!! My friend was in the program and she said it was tough but very rewarding. Enjoy your freedom now.

Hey Mairayhans, Thanks for your reply! I am fully aware of the difficulty factor that these programs consist of since they are short termed. I know they are ranked 28th in the Nation but that is for Graduate level studies. I expect the program to be rigid and extremely exhausting but most of those earlier posts say that it was very doable and they do mostly stimulation labs (which are disorganized) and less time in hospitals. Hopefully things changed in the last year or so...for the better. I just have my worries of spending 6 figures on an education that I could have gotten from a public university for 40,000 like everyone else :/

Ah I see where your coming from. Well then I think you should stay in-state since your in transition of becoming a Florida resident. My GPA for major is 3.51 and my pre-reqs are 3.7-8 depending on what the school looks at. I've received acceptances from Marymount University in Arlington, Va along with Umass Amherst. I am waiting for NYU right now as well as FSU. I will definitely not be attending FSU.
