Patients vitals- 119/60 hr-74 o2 stat- 93 on room air, rr-18 at resting, temp 36.7 Crackles in lower left lobe Nonpitting edema bilaterally in lower extremities and unilateral left upper extremities Pain- 5/10 radiating from posterior chest to anterior (left side near lung)- comes and goes with movement -patient states "throbbing, and it feels like someone is nagging me all the time." Aox3 cough- non productive but causes sob (has asthma as well) Braden- At risk for pressure ulcers- kyphosis keep an eye 16 Hendricks- High risk for fall 6 Katz- Patient is independent 5 (needs help getting up) Uses cane to steady self when ambulating; good ROM in upper and lower extremities hx of pneumonia- has the influenza shot but not pneumococcal Sorry running to class. Risk for falls r/t impaired physical mobility due to pain and peripheral neuropathy. Readiness for enhanced nutrition r/t a desire to improve wellness evidenced by expresses willingness to enhance nutrition. or Readiness for enhanced self health management r/t a desire to improve pulmonary function evidenced by desire to manage the illness
  3. Patient is 78 History- COPD, HTN, Osteoporosis, Peripheral neuropathy, GERD. Came in with pain in lower left thoracic cavity- thought fx ribs but nothing on xray. Pneumonia diagnosed 2 days after being admitted.

Risk- risk for falls rt impaired physical mobility evidenced by pain in lower left thoracic cavity and peripheral neuropathy. Health Promotion- Readiness for enhanced nutrition rt desire to improve evidenced by low calorie intake but expresses willingness to enhance nutrition. This is what I had at first but my patient did mention stuff about the pneumococcal vaccine. Especially since she has a history of pneumonia. But I was having difficulty figuring out what to write/goals/interventions.
  5. Hi all, I am currently working on a nursing care plan and am stuck unfortunately. My patient has COPD with pneumonia. So I wanted to do readiness to enhanced immunization status r/t desire to improve health evidenced. I am having trouble coming up with interventions. short goal- Patient express knowledge on the importance of vaccines esp pneumococcal. long goal- Patient receive recommended immunizations yearly... Interventions- Short: Assess patient's perception of the pneumococcal immunization. Educate the patient on risk factors of receiving the pneumococcal vaccination. Discuss the risk factors that increase the likelihood of developing pneumonia. Long: Having difficulty with... Any suggestions? Thanks!!

  7. Hey Guys! I just wanted to see if I am on the right track for a nursing care plan also I don't know about the long term goal (if its any good)... Thanks for your help! K Actual: Impaired skin integrity related to malnutrition and physical immobilization evidenced by stage 1 pressure ulcer on sacral area. Short term goals: -Patient will maintain skin intact with no signs of further breakdown by change of shifts. Interventions: -Frequently repositioning the patient every 2 hours or as patient requests. Rationale: "Reduces the duration and intensity of pressure." (Perry, Potter, Elkin, 2012) -Assess the patient's nutrition status Rationale: "Deficiencies in any nutrient results in impaired or delayed healing." (Potter & Perry, 2013, p. 1184) -Monitor patient's incontinence status and minimize exposure of moisture to area. Rationale: "...exposure to gastric and pancreatic drainage has the highest risk for skin breakdown. " (Potter & Perry, 2013, p. 1189) Evaluation: Hypothetically, Long term goals: -Patient will verbalize the importance of good skin integrity by discharge. Interventions: -Educate patient on the importance of nutrition and eating a well balanced diet. Rationale: "A balanced intake of various nutrients (i.e., protein, fat, carbohydrates, vitamins, and minerals) is critical to support wound healing." (PP 2013, p.1186) -Educate patient on the importance of hydration. Rationale: "dehydration can affect albumin levels and tissue integrity." (PPE, 2012 p. 613) -Patient will tell me in detail what is the cause of breakdown in skin. Rationale: Evaluation: Hypothetically, I would teach the patient the interventions needed to protect against further skin breakdown and promote good skin integrity.

  10. Any people who have been accepted already who taken pathophysiology? Im still waiting on acceptance but just wanted to see if NYU takes this course as credit. My other school doesn't and requires me to take it over once I am in the program...
  11. Yeah I don't think priority made that much of a difference...Still waiting here as well. Did anyone receive financial aid yet? I wanted to see if they actually offer money?

  13. BrooklynJulie-- I would be soo ****** especially since you physically handed everything into the office. I called around March and they said everything was in for me and I sent everything in around the end of April. I didn't get notification yet either so we shall see. Hopefully that doesnt hurt your chances!
