I think you absolutely did the right thing. I started working at an ltc/rehab during school once I got my lpn and continue to work there prn and I think the most important lessons I learned were to do what I believe is best practice and best for my residents. I always check my own orders, consult with the doc when appropriate, and consult with the residents to ensure they are getting the kind of care they want and deserve. I gladly accept the advice of the other nurses but in the end the residents assigned to me are my responsibility. I agree with the pp, I would love to work with you and as for nurse j she is completely out if line IMO.

So who do nurses marry or have as civil partners???
My husband works for an oil company drilling wells, he works long crazy hours so we are used to the other one doing shift work.

Patient's right to refuse
I agree if he wants to refuse absolutely everything then he can be discharged. It is very frustrating to have patients that want to refuse (and sometimes argue) everything! I always wonder why are you here, if you just want to do whatever you want against the advice of healthcare workers then go home and do whatever you want!

I can't believe that I made such a stupid mistake and am kicking myself!!!
When I was orienting I forgot to waste once! I pulled from the Pyxis withdrew some morphine and promptly popped the vial in the sharps, I didn't even think about it until the next day. I let my preceptor know and she said not to sweat it but I felt horrible, I ended up reporting it to the director. One "event report" and drug test later I was ok and felt better about the situation. I have made a lot more dumb mistakes and every time it eats me up for a little while, but as pp said, I don't ever do it again!

Post-op PTCA tips?
What are the steps in sheath pulling? I work on a cardiac step down unit and I haven't had to pull one yet but I'm nervous for when I do! Any tips?

Mistakes
I am a new grad, started feb 4 at a local hospital on the cardiac stepdown unit. I was incredibly honored to have a chance to be on this floor. However, I feel like I make so many mistakes and it causes me major anxiety and stress. Last night when I came on shift one of the patients had accidentally pulled a wound vac out, the day nurse told me she talked to the doctor who told her to remove only the black foam and do a wet to dry dressing. Fast forward to later on my shift, my patient spikes a temp, I call the doctor who orders a work up and iv abx about an hour later my patient is looking better and states she feels great, no significant changes with her lab work. Fast forward again, I am giving report when wound care arrives, they are completely ticked the white foam was still in cuz that is a big no no! I feel truly horrible and like I caused my patient to get that fever and like her health may be compromised because I didn't question more and it was in my entire shift. I also feel like a failure and what if they fire me for being so incompetent. It's just completely eating me up inside...

Wasting
I am a new nurse and got busy the other day, I withdrew some morphine out of a vial and completely forgot to waste the remaining, my preceptor told me not to sweat it but I feel nervous about it...any advice?

Nclex tomorrow 1/14
Hi everyone I already took nclex and passed but I was wondering what the "good pop up" is? Good luck to everyone and try not to stress too much! I had over 150 questions and still passed, hang in there!
