I need help with my documentation. I have a patient with hip pain. The pain is in the right front at the hip area. So to me what I am documenting sounds stupid, so I need help. This is what I put: Pt. C/O constant pain @ Anterior Right hip X 3 days. + Clicking, +Popping, - inflammation/redness, - grinding. Pain 7/10 on pain scale. Ibuprofen has not helped to relieve. Is it right to say anterior or is there another word to explain location? What if she would have pointed to her side or her posterior area? Does that cover what I need to cover?

I am aware of what I SHOULD do, but due to circumstances that may not be possible. My question was, Is it considered abandonment? Thanks pricklypear and tiffany. I was aware of group one. That was what I was concerened about. Ugh.... I found the list of current clients Groupone has. It is not as many as I thought it would be.

OK, I am a new nurse and honestly don't know the answer or the consequences. Saturday I gave my 2 weeks notice at my previous employer. The reason I gave 2 weeks is because I thought it would be considered patient abandonment if I didn't. Someone told me today that this is not considered pt. abandonment. I don't know what to do, I am leaving for many reasons but mostly because I feel like my license is at risk here. She has me on the schedule for the next 2 weeks, what are the consequences of not fulfilling that 2 weeks notice? I did give it in writing and we also talked verbally.

Like I said in my previous post. DO NOT WAIT to take it, take it as soon as you can. Studying is good, but alot of what I was tested on was what I was taught in school. The longer you wait, the more of a chance you will forget or not comprehend or something. Our school has had a 100% pass rate for the last 4 years. They highly recommend you taking it right away. I know how stressful nursing school is, it is awful. I have 4 kids and literally had to shove them away for a year, it stunk and killed me, but now I am trying to make up for it. We all knew going into it how hard it would be, but it isn't until you get into the middle of it, do you realize how hard it actually really is. What is your school's passing rate, have they told you that? Also where are you at as far as your grades go? That doesn't always matter, but can be a good indicator of what areas you need to study on. Does your school offer you a Mock NCLEX test? If so, that will also tell you where you are at as far as the national standard goes. Good luck and don't wait, "get 'er done" right away.

They would have been doing CPR on me if I would have gotten all 205. At 85, my heart was pounding and I was get SOB, I felt like I was going to puke.

Wahooie, I passed that sucker. I took it on the 12th and got my results around 11 on the 14th. My bestest buddy and I both took ours at the same time, both got 85 questions and both passed. Whew!!! HOOT HOOT!!!

I started studying in January, just going over the NClex, trying to get a gazillion questions under my belt. I started studying very serioulsy Mid September after I set up my test date. OK, this is my 2 cents...no amount of studying will prepare you for what you will be asked, but you need to know your basics. It may ask you a question about Alzheimers, but what it is really trying to see is if you understand Pt. Safety or get Maslows Hiearchy. It is crazy crazy crazy. I took mine the 12th and found out yesterday I passed. I got the minimum for the State of Texas which is 85. One big tip. Take your Boards as soon or pretty quick after you graduate. Do not sit around and study, because you will feel unprepared or like you have lost some of your umph.

Oh gosh how awful, she will be there for you and she is right, you just have to take it again, there is no doubt about it. It is going to take you some time to get inspired to do that I am sure. Give yourself some time to deal with the shock and let yourself "grieve", your self esteem has just been blown. You know you know this stuff. Do you know of anyone else in your program tha has had problems? How did you do in school? Our school made us buy a membership to a website called medspub dot com It had a gazillion questions on it. Also you may want to try to get some nclex study guides off of ebay that is where I got mine and I got them for way cheaper then in a regular book store.

I don't know if I have any advice or not, because I am not sure I passed since I just took it today. But I know how I will feel if I fail. I almost think it will be worse to tell my family I failed then it would be to actually fail. Failure is not my cup of tea. Someone told me to do atleast 100 questions a day and make sure you do 3000-5000 before you test. I think I got in about 3000 questions using all different types of books. I used alot of different CD's so I would get the computer practice. I also gave myself a couple days break before I took the test, so my mind wouldn't be so bogged down. But I was still a freakin wreck going into it this morning. This is your dream right? If so, it is worth it. Take a few days off and then get back to it. I am so sorry, I wished I was there for you to hug your neck.

How on earth did you get 82 questions? Minimum is 85... Congrats anyhoo. I sure as heck hope I get mine back that quick. Please Please Please!!!

CONGRATS!!!! HOOT HOOT!!! how long did it take to get your results? Didn't you just take yours yesterday? Did you take it through Pearsonvue? I am trying to figure out long this is going to take. Some in my class has gotten theres back in 24 hours, some have gotten them back in 48. I want mine back now!!!!! LOL

After much thought and consideration, I am looking for another place of employmnet. I cannot continue to work here as I am still being chastised for this event. I had yet another nurse come to me and tell me we never document like that and what do I think Medicare would think if they saw that kind of documentation. It made me sick to hear her say that, but I asked her what does she think medicare would have thought if this patient would have coded? Does she expect me to cover for another nurse? She said yes we do that around here. We take care of one another. I told her, I am here to take care of the patients. She said to me, "you will learn real quick we take care of one another. My stomache flipped and flopped after that. I had another situation happen that night where someone took one of my patients off for me while I was taking off one of my other patients. My pre-ceptor came to me and asked why the tech did it and I told her I was unsure but I thought because she was working as a team member she was just doing what we do. She told me that I was wrong for letting her do that. I coulnd't believe what was coming out of her mouth. If someone is willing to help get patients off then let them, we all try to help one another (those of us that work as team players) she continued to drill me on this over and over, it was almost like she was trying to get me to blow a fuse or something. I threw my hands up in the air and said, " I am not going to argue with you over something like this", walked up to my charge nurse and asked him if he had anything else I could help him with and he said, nope, looks good, you can go home. I walked out in tears and cried on my best friends ear all the way home. Started looking online and in the paper and faxed several resumes out. I have gotten great response, had an interview on Friday, they called me back an hour later and asked if I would come in for a second interview. I was thrilled because this position fits me, my beliefs and my family to a T. Crossing my fingers for a successful interview.

Not around here (Tx). But I also wanted to mention something gross. My mom got MRSA in her Belly Button, for no reason at all. It is weird but true.

Whoa, that is scarey. I am with someone who said earlier, you don't want to work there anyhow. Why would you give a med someone else had pulled? As nurses we know better then that. Never ever ever do that. BUt, this seems harsh, the punishment does not fit the crime at all.
