  1. ABSOLUTELY NOT!!!!!!! Duragesic patches are made to dispense the medication transdermally over a period of time usually 72hrs. Cutting the patch will never ensure that the patient is getting the correct dosage. Also some duragesic manufacturers make the patches with the liquid medication visible in the patch and cutting it would release this med. The solution to the problem would be fill the RX for the 12.5 mcg patches. Apply 2 patches in the same area so as not to froget to remove them and change both at the same time. Cover the entire patches with the tegaderm to ensure that the patches do not fall off.
  2. I agree! I think this an unsafe practice and I would not place the patch on the patient using this method. I think your solution of applying for 25mcg patches, dispensed as 12.5mcg is a great idea.
  3. I used the Core curriculm study guide and I found that helpful. I also had a disk that was study questions that I found helpful. I did take a review course but it really wasnt helpful. I found that just doing the questions was helpful. Good Luck!
  4. I work in a big Cancer Hospital. The policy was just changed and we are now required to wear gown and gloves to put up and take down chemo. I think its a great change and that there should be universal practice that every facility follows.
  5. Just wanted to give you a bit of encouragement! I am also just off of orientation and am going thru hell it feels like AHHHHHHH. I am exactly off of orientation and have work 3 shifts. My third one was horrible. I even started crying (at the nurses station) when approached by a superviser... How embarrasing! I feel the same way you do. There are days i even question why? After all this time going to school ... I never thought i'd be stressed like this! Do I really want to do this? But then you'll have that patient who says thank you, or you feel that you have made a difference even if it is small, somehow you helped someone. I want you to know we all have bad days. Even nurses that have been doing this for a while have a hard time. You worked hard to get where you are dont give up! These days will just make a better nurse out of you! I feel your fustration. Try after each shift...think of one thing you learned or did that day....that will be easier tomorrow... And last but not least...remember you chose nursing for a reason...I believe it will get better....Pray for strength and hang in there!!! :)
