Wrong patient. Wrong narcotic.
Our supervisor went behind the nurse still looking at the wrong patient and agreed it was missing. The PCP was called to ask if we could apply a new patch. Shift change happens and no patch was reported missing. The doctor called back informing the night nurse saying to apply a new patch. The day nurse was called and she admitted she forgot to say it was missing. The night nurse applied a new patch without looking for a old one. The next day an aide realized there was two patches on her one on each deltoid. That's when they realized the day nurse was looking at the wrong patient. The night nurse is being written up for not changing the order in the MAR for the patch and applying a new one. The day nurse isn't getting written up at all. Fair?

Could a DON please input on something for me? There was a nurse at my facility who thought a 100mcg duragesic patch was missing she checked the wrong patient and ordered another. She left without saying it was "missing" in report. Everyone else in the situation is getting written up. Shouldn't she since she checked the wrong patient? Doesn't that violate the 6 rights?

I have family down there who I can stay with until I do find something. Are they wanting 1 year + experience in California? I do something's pretty independently j-tube wise and IV wise.

question regarding meds
I have never stayed hours but an hour after my shift yes. Sometimes it is charting. I have 30 residents to myself. No med tech no other nurse. Only 2 CNAs too. It is crazy. Mainly though it's charting or if something happens. Giving report. Taking inventory on meds the night that pharmacy comes. Anything can throw a person off for example. Having a dementia PT who refuses her meds and keeps you in her room bc she is paranoid.

Thinking of moving I was told LVN employment in CA is high? I have 6 months experience in LTC between our SNF and our rehab unit. Idc where I would work. Thinking of San Francisco area. Any ideas?!

I had a girl I went to nursing school with who made a similar mistake she simply would lie about the experience and after a few years no one would call. I am not sure if this is what you want to hear but I hope it helps!

3rd Day of Orientation and I hate it
Sometimes it is. I work at a LTC facility and I float between our 4 wings. My first wing LOVE it. The nurse was great at making sure I knew what was up and when I messed up the narc count she was wonderful. On another unit the nurse is ridiculous. She was rude. Laughed if I didn't know how to do things right. I didn't do a G tube med admin and flush quick enough so she took over. I wanted to pull my hair out with her she was just so rude. Just wait until you have the role by yourself. Everyone has diff methods no matter what field you are in and some nurses are just rude to new nurses. Best you can do is ignore her until you have it all to yourself. All the things you mentioned doing is totally normal on a shift. It seems like a lot. Sometimes it is, it requires working through your break but you have to find a system that works for you. Hope this helps!!

Bad CNA staff, any advice?
First off. I would like to state I was CNA before becoming an LPN. I currently work 3-11 during day shift there there is a nurse 4 CNAs, one med tech, and a restorative aide. During my shift there are 2 CNAs and me. I'm experiencing tons of complainants from first shift on not changing residents I offered to start running this shift. How do you keep unruly CNAs in line?

I feel exactly the same way. I have been working at a SNF for 3 months. I am a new grad. I enjoyed LTC during clinical but I had a hard time with death and didn't think I could handle it. I originally only took this job bc my husband and I needed the money. However I love it. I have am amazing supervisor. I love my patients and even after a rough day I love knowing I made a difference.

Time Budgeting.
I am a new grad nurse with my first job in a LTC setting working 2nd shift though I am the only nurse on my hall. I find it soooo hard to go ahead and get meds around when everyone seems to be on Narcs. I know I shouldn't but sometimes I sign them out before I actually give them. Any tips to where I can not do that? I also do no socialize really and my CNAs are good so it's not a matter of babysitting them either.

Sleep Schedule
So I have just gone from a 1st shift job to a second shift one. I need some help. It has messed with my eating and sleeping. I'm going to be on this shift for some time, you have to stay 90 days in a shift before switching. I get home close to midnight and go to bed around 1. What is a good schedule for eating and sleeping?!?!

I'm in KY as a new grad at 16hr and weekends 20/hr I'm in LTC
