Heartmate II Patients and CPR
I love that device, if I were to get one it would be the heartware. My patients mom brought in his power stuff for me and had the car charger and I was super entertained, I had heard about it but not see one. Really they can go to AC power with what looks like a laptop cable.

First question: We check to see if it is a controller problem by listening, if it is replace the controller. If not treat arrhythmia/volume etc. All while someone is calling the mechanical heart team (RNs or engineers) and the surgeons. And only when they say so we do compressions doesn't mean we aren't doing the other parts of ACLS in the mean time. Second question: When they do well they do great, I can think of one that came in once during the year and a half I have worked on my floor, another 2 that have come in twice and are out boating/riding mopeds. But then there are those that just do terrible and we get a little bit to much exposure to that in the in patient world. We have some awesome cases were it gets them to transplant and then some heart breaking stories that still have left me unsettled since April. Bottom line, its a new field and each new device is getting better and we are learning how to better treat the patients and the device.

Dating Patients
Never thought I would ever find any patient attractive. A couple nights ago I did have a very pleasant admit on a nerdy (my kind of cute) younger patients. I very rarely like admit but with most questions he would have a story or two some about people that we both knew (former patient and his wife/new coworker) and about life in general, his nephews, strokes-his and my moms came up when I told him that I understood his frustration and that things do get worse when your exhausted. Also just in doing his physical assessment his wanting kids in the future came up, being able to relate to their acne. The whole night I did his assessment, a bedside swallow, the history/meds, vs x2, nuero check and got him a scrub top (totally against the rule by the way but I will do it for any patient no just the cute ones). Anyway did I say anything to him that I thought he was cute no, will I do anything to pursue this, no. Would I object if I ran into him outside of the hospital a bit down the road, no, would I immediately have a physical reationship, no not with anyone.

Hgb 7.2 from 13.3 after delivery
Just as a heads up I am not an OB nurse but found my teacher fascinating and she drilled a bunch in to my head. But you have to realize that while 7.2 is very low in the next day or two postpartum women diaeresis a couple litters and that will bring it back up into the 8-9 range. And I work cardiac and unless the patient is dramatically symptomatic we try to find the cause and support them more than transfuse (especially with our transplant listers).

EPIC Computer Program
Does anyone have comments about whether or not real time charting has to happen, because as night shift it is nice for patient sanitation if I can assessment as quickly as possible and then chart at the desk a bit later.

Pushing 0.5 ml med into a central line
I totally agree, this is how I give everything that doesn't have to my dilluted, and really push the benadryl then go slower on the flush.

guilt over forgetting something?
SO I just worked a 12 hr noc in the ACU, besides the middle which was pretty busy it was good. At 0600 ish another nurse's pt started vomiting what looked like stool, severe back pain, central tremor, chills and fever. So myself, the primary RN and the rapid response RN were all in there for the better part of an hour trying to figure out what was wrong. Well my pts were all sleeping. But I forgot synthroids for 2 pt and a cardiac med at 0600. My plan was to give it about 0630-0700. And since I realized it I fell super guilty, I know that we are all human and that the next nurse will notice the that it wasn't give and it will be technically 2 hrs late, but no harm should come to him. And tomorrow I will apologize to the day RN. Does anyone else feel super guilt if you forget things or is it just me.
