When to refer to social worker?
Hi everyone! I was wondering if I could get some advice regarding when to refer a patient to a social worker. I currently do the admissions, just started home care in June. I'm struggling when it is appropriate to refer my patients for a social worker. For example, I had a patient today who is elderly, lives alone in 2 story condo, no children. Drives, but not often, probably shouldn't even be driving in the first place....but anyways, we are in for care to ulcer on her foot. Unclear when I asked who does her food shopping ect...declines HHA. Appropriate for social work eval?

Another recert?
Hi guys! I have yet another question with recerts ..... so I called the doctors office to let them know that I was keeping their pt on services and explained why. The assistant/secretary or whatever said " ok..so what do you want me to do?" -very rude. How does this work normally? I called the office, told them we would be keeping their pt, gave them a brief update..should I have done something differently?

Safety- in HH vs. Hospital
I recently left the hospital that I worked at for 5 years, and I have to say I never really felt that safe! The detoxing patients that spit at you or attack you, confused people determined to get out of bed at any cost....no thanks! At least with home care, people want your help and are a little more appreciative.

Medication Reconciliation
I am just starting in this field and I am kind of confused when it comes to reconciling meds. What exactly is med reconciliation? It is the meds that the next pt has, that don't match my list? Or vice versa? The program has a section for "list of meds to be reconciled" and im not sure what I am suppose to put there.

Per Diem Question....
I am looking to leave the hospital I have worked at for the past 5 years to become a home care admission RN. I am going to have to give my two weeks notice, however I would like to stay per diem if they will have me. If I plan on staying per diem, do I still give my two weeks notice? Thanks!

Traveling in hospital with the patient
Any tips on how to make traveling to CT scan/IR/Nuclear Medicine within the hospital with an ICU patient who's vented and on all sorts of gtts? I have been a nurse for 1 yr, and have been on nights. I am switching to day shift and I am having some major anxiety thinking about traveling with these patients....

I have a question: With a swan, when obtaining a CVP reading, you turn the stop cocks up and down ( left up, right down).. Why do we move the stopcocks? I guess I'm just looking for the rationale behind this..OR maybe I'm just over thinking it..

Graduating in 2 wks - terrified!
I graduated in 2010, and I know EXACTLY how you feel! I was a CNA in an ICU/CCU while in nursing school and as soon as I graduated, they hired me in the ICU as a new grad.... I have never been so terrified in my life. I had such anxiety those first few months, let me tell you. I nearly had panic attacks going into work, and leaving worked my adrenaline was going to bad I couldnt even sleep. And I did make a few mistakes, but I realized them and took the steps to fix them. Your only human, and every nurse will make mistakes. Just be glad they none of them were truly life threatening, and take comfort in the fact that you've learned from those mistakes!

night shift depression
I have the same problem! I've been working night for four years now and I am currently looking to get off this shift. I've been getting pretty depressed and I sleep forever sometimes! And it sucks on my "days off" ( which never really feel like days off) I have to get up on like 3 hrs sleep so I don't waste my day off! I feel your pain...
