  1. I don't think you need to put it in your application however I would suggest that you definitely mention it in the interview - so as to legally cover yourself - verbatim is considered legal. Instead of calling it disciplinary action - you could state that an issue came up - what it was - and that you had a positive 'learning experience' which has in turn improved your practice and ensured you now consistently monitor your nursing practice for ways in which you could possibly improve it. Let the potential employer know that initially it was thought to be a disciplinary action - but actually it turned out to be a positive experience and that this is why you did not include it in the written application. Sorted.
  2. I definitely wouldn't quit over that. You have worked hard to get where you are and deserve to give yourself a desent chance at this career! If I was in this situation, despite my ego, I would enlist the help of someone senior at work that can thouroughly go over the error's with you, perhaps find a trend and then look at techniques to stop this from happening again. You have already taken the right step by admitting you have a problem here - so now take the next steps to right the problem. And remember - errors/mistakes can never be eliminated but can be minimised when you are practising at your best.
  3. Totally sympathise with your situation.....has happened to me before. Maybe you could look at being more assertive with your colleagues. Always ask for help when you need it....we are suppossed to be team-players and you can never 'get in trouble' with the boss for simply asking for help. Good luck!
