Took the Kaplan diagnostic exam.. I've got 72.78%.. is this bad?
So, I'm confused too then. I got a 56% on the diagnostic. On my exit HESI I got well over 1000. Which is a better indicator?

Update on my post last night. Took my exit exam today and got 1000. My school requires 900 for graduation! Woohoo!

Man! my school has been taking HESI tests since day one of nursing school. 2 a term, and we had to average 75% or higher to pass each term. we started with 65 students and will only graduate 34 in a month from now. we also have to pass an exit exam, but at this point we are all so familiar with the testing style I'm sure it wont be that difficult.

Thanks so much!! Here is another oddity that makes no sense. I know there is potassium depletion due to frequent urination, but why do labs usually indicate hyperkalemia?

Ok, I'm trying to understand the underlying pathophysiology behind why dehydration happens with DKA patients. I understand that there is an increase of glucose in the urine, and an increase in urine production, so obviously the need to urinate is substantial. Is this what is causing the bulk of the dehydration?! Thanks

Nursing assessment for pt with possible Pulmonary Embolism
Pt's vitals were somewhat stable upon admission, however over several hours bp dropped to as low as 88/58, and pulse, resp, temp, and pain levels all increased. O2 sats tanked from 90% to 80% while on 2l/nc. She was in sinus tachy, and began coughing bloody sputum.

no pt hx of afib, dvt or coagulopathy. only allergy is Sulfa meds, and currently on the pill. 1 ppd smoking for 8 years, and a recent trip from Asia. I suppose the long flight might have been a precipitating factor.

For my ER rotation I had a pt present with s/sx of a PE, dyspnea, SOB, pale, sweaty, pain 7/10, and decreased LOC. upon assessment I obtained new vitals and pulse ox, full pain assessment, lung and heart sounds, ECG, chest x-ray, ABG's, UA and sputum sample to rule out possible infection. However I felt like I totally missed something. Also, when it comes to getting a proper medical history, I obtained allergies, contraception, smoking hx, and recent trauma. Is there anything else that would be beneficial to know other then this regarding pt hx? Thanks:nurse:

I really appreciate the help! I wasn't trying to get anyone to do my homework via this thread, I was simply asking for suggestions to get my mind working in the right direction. As a first year nursing student with NO OB experience its difficult to know the priorities regarding L&D and postpartum care!! Thanks for the push in the right direction!!

I guess I'm just confused then. The title of the case study is "Normal Childbirth" No tearing, no episiotomy. She did have 10mg Nubain IV for pain, but other then that I am not seeing any complications. In our case studies we go through the top three nursing priorities, although with this gravida 3 para 3 I am not seeing what else needs to be done. I guess there is always the possibility of postpartum complications (hemmorhage, constipation, depressions) but nothing is addressed in the case study so I have no idea what direction to take this.

HESI during Nursing school?
So, do all nursing schools make you take HESI's? We have two a term, and have to pass with a combined total of 75% to continue on. Even after reading Kaplan, and Saunders I still have no clue what the heck I'm doing. I have taken 5 so far, and I'm just as lost as when I took my first... I read that a lot of schools make you do a HESI exit exam, but we have two a term for 2 full years...Why?
