Can someone please explain the pathophysiology as to why a diabetic patient's blood sugar will elevate when they are NPO, wouldn't it be more likely to drop. I know doctors will order sliding scale for coverage and I am thinking long acting insulin should be held? Thanks.

Hello, I am a nursing student and hoping someone could clear a few things up for me. First, I am terrified of causing an air embolism via IV line - I know to prime tubing to get air bubbles out - but when it is time to flush a line the pre-filled syringes always have an air bubble in it - if I push the bubble through won't that cause an air embolism too? If I don't push it through then the patient will not receive the 3cc's there supposed too. Secondly, they say IV tubing at my clinical facility is good 24 hours - but let's say the patient has IV piggyback I am ready to hang the second dose - the tubing has not yet expired but won't re-spiking the bag with the same spike introduce bacteria? Should I be getting all new secondary tubing each time? Lastly, I am getting confused on setting the IV pumps. Let's say my patient is getting piggyback antibiotic 50 ml/hr and the total bag to infuse is 100 ml. I understand how to set the piggyback - but I know they told us to run NS after the secondary dose is finished to make sure they have received all the medication - at what rate and what amount to infuse am I setting the primary bag? I was also taught to always clean cord with alcohol with each diaper change. If you were taught not to use alcohol, then what did you use as alternative?

I am from Massachusetts, my friends starting wage are about 26-28.00. I am in Raleigh Nc, starting wage at doctor's office is 18.00 - just got a job at hospital making 22.50/hour, plus with my shift differential - third shift - making 29.50.

ROME Respiratory Opposite Metabolic Equal Example: Normal Values: pH 7.35-7.45 PaCO2 35-45 mm Hg PaO2 80-95 mm Hg HCO3 22-26 mEq/L Lets say pH is 7.50 CO2 32 CO3 24 THE PH is elevated and Alkaline THE CO2 is decreased THE CO3 is normal So the pH and CO2 are going in opposite directions which = Respiratory Opposite so this would be Respiratory alkalosis Now lets say the pH was 7.32 (decreased and acidic) and the CO2 is 33 (decreased) since there both going in the same direction (decreasing) = Metabolic Equal and you would have Metabolic Acidosis So basically take the Ph and determine if its acidic or alkaline then take the abnormal CO2 or CO3 value and determine which direction they are going (elevated or decreased) and apply ROME. I live in NC the first time I took the NCLEX I failed and Pearsonvue brought me to the CC page. The second time I took the test with 75 questions and then I got the popup box and passed~~!! I have a friend that lives in MA he got the CC page but he passed. So I guess It's not always accurate. I feel bad for all you CA people I can't imagine having to wait that long. I am a new grad and made a resume that included my work history and schools. I realize now it should have included a "skills" section - but its hard to fit everything on one page. I emailed my resume to a doctors office and recieved an email back stating "You need to put where you did your clinicals and what you did clinically at each on your resume" So now I'm a little confused. Should I put down everything real specific like "Interpreted EKG strips" "Ambulated client with gait belt" doing it this way the list could get pretty long. Or am I supposed to do more broad - It's not like I have a ton of experience I just graduated - and is this supposed to be on my actual resume with job history - Or a whole new separate page. Someone please give advice - I need a job!!! Not sure what exactly this lady is looking for. Thanks!

I know exactly how you feel. I just graduated and accelerated ASN program. My school was soo disorganized everyone was failing and there was no one to answer our questions. It was very depressing and everyone was miserable. I was close to failing as well. It was so hard but I kept with it and didn't give up - I just had to dig deep and somehow I found the strength. I wasn't sure I even wanted to be a nurse at that point but I had already sunk in so much time and money and commitment I felt I had no choice. I'm not sure how much schooling you have left - but my advice is to stick with it and it will be that much more rewarding in the end. I'm happy I stuck with it!! Good Luck! :paw:
