4 : 1 nurse patient ratio for med surg???
WOW!! on a regular night it was 4 Rn's on my unit with7- 8 pt's. each and at times 9 on really short nights one or two of us had 10 the other 2 had 9... (depending on the census.)..generally 2 nursing assistants on the floor..On really short nights we have worked 4 RN's 34 patients on the floor 1 cna...on lucky low census nights we had 6 each but that was pretty rare and ER would fill us right up!!

Career ending mistakes?
First let me say that i've had 80 year olds that are like magicians and can get out of a posey vest or wrist restraints in a heartbeat!!As nurses we do our best to keep our patients safe and especially the confused pt. who climbs out of the bed..I have seen pt. falls even when the pt. was across from the nurses station..and I have had 9 pt. with at least 2-3 with periods of confusion and on fall precautions..and yes it's difficult because I can't be in 9 places at once.. frequent rounding, bed alarms,safety restraints, help from other staff such as cna's and other rn's on the floor, and when possible a sitter are ways we can prevent/reduce the number of pt. falls...I always was told by my mentors to have your own personal insurance it's a good idea to look into that...the annual premium is not bad under 100 bucks for most...

I generally immediately remove the iv, elevate the limb and apply warm compress.

Is there ANY department in nursing that I can avoid cleaning up poo?
LOL LOL not only is there no nursing dept. thatI know of that doesn't deal with poo you will be dealing with poo as a student and for sure as a nurse..It is a misconception that only cna's do all the poo cleaning.. they are wonderful assets to healthcare and they taught me sooo much when I was a new nurse without a clue how to turn and clean a pt. with c-diff covered from top to bottom with poo..I was humble and learned alot... never think you can't get your hands dirty because it's all about teamwork so strap on those gloves, a gown and at times a mask and get to cleaning because once you give off the vibe that you can't help out your job will be ten times worse because noone will want to help you..good luck

I hate the NCLEX-RN Exam
sorry to hear about your test hope this helps..remember you must choose the best answer..two of the answers may be what you are going to do.. but you must think as the nurse what will you do first..ex..if a patient falls on the floor yes you as the nurse are going to call the Dr. but first you are going to get the patient off of the floor and take at least a bp before you run to call the Dr.... NCLEX tests that you as a nurse can use critical thinking..and also that you have knowledge to provide safe competent care at a basic nursing level.. also i think if you don't pass they send you a breakdown of the areas you need to work on..review those areas and practice questions in those areas.. don't worry if the test goes over 75.. I passed the first time with more than 75 ques.. I know someone who passed at 75 ..don't get too caught up in the num. of ques. Good Luck

Reprimanded for NOT giving IV Push Medicine
You did the right thing for refusing..at my facility in order to push anything iv from lasix to ativan the RN has to take a special class and pass.. The nurses who didn't yet take the class call the resident at my facility to do iv push meds ..I will not risk my license or the patients safety..and with something like IV push ativan depending on why it's being given it can be considered a medical restraint ...always practice within your scope it protects the patient and you..

How long is a spiked IV bag good?
To clarify my response(I didn't read the ques..fully) I have never seen a facility do what you are describing that they do for a flush!!!I strongly suggest that you do not do that..You never want to draw your flush that way ,the contamination risk is very high!! and I would be written up had I ever gotten caught doing that....joint com..would surely not condone that practice..Originally I thought the ques. was about hanging iv fluids on a patient..my first ans.addressed that.... but the flush for all to use is not wise..

A spiked IV bag must be changed every 24hours..the tubing is usually good for 72 hours...

Things you'd LOVE to be able to tell patients, and get away with it.
Oh yeah and this is for the patient who is ambulatory but is lazy at night and rings the call bell for his "nurse"... empty your own freakin urinal!! I am not your maid!!! and family members too!!! they aren't excluded ...This shout out is for the know it all family members who insist that their demented love one who is barely able to swallow, and will most definitely aspirate any thin liquids have a cup of water and a few bites of food (not puree )while you were out of the room... and before you know it your patients lung sounds are congested as ever...you dummy he/ she cannot eat/ drink that.. you don't see that they are damn near choking!! boy that would be nice to say!!! ok I'm done LOL...

If you ring that call bell one more time I will break your fingers!! No we don't have any more juice go to sleep the damn kitchen is closed!! Stop bending your damn arm thats why the machine keeps beeping!! stop complaining and just hold out your finger !! Take your butt to sleep and stop worrying about who's talking at the nurses station! LOL...to name a few!!!:chuckle

How would you make inpatient rooms better?
LIGHT LIGHT LIGHT please!!!!! I am tired of squinting or having to disturb the other patient in the room and turn on all the lights in order to start an iv at night!!!!!

How did you react to your first code?
My first code was actually a lil traumatic because the patient became unresponsive basically with no warning...it can happen ,talking one minute gone the next.. I participated by handing the Dr. what they called for, atropine etc..but when I left that morning I went home called my boyfriend and cried like a baby..I was sad for a few days..I think it is different when you are watching and curious as a new grad... but when it is your patient that codes I think the experience for some nurses is a lil different..over time I have now seen quite a few codes, but the first time was a sad scary experience for me..

75 Questions on NCLEX, pass or fail?
New grads don't worry...I have seen both ends of the spectrum... I passed the first time and I had more than 75 questions..My girlfriend failed and had less questions than I did..I can't remember the number...study your weaker subjects a lil harder my weakness was psych..so I focused a lil more on that but the test can have anything from diet to med calculations..mine had quite a few maternity, psych, and diet questions I had like 2 or 3 calculation ques..it may be different now..The day of your boards eat a good breakfast and don't study !!have a cut off point the night before.. If it gives you peace of mind leave your notebook or text books in the car so after you finish you can check for the ans. of the questions that you guessed..don't worry you will have to guess on a few but remember to think about what the NURSE will do in a situation rem..ABC's first careful if it says what is the nurses FIRST response of course you are going to call a Dr. in an emerg.. but if patient is on the floor you are going to pick the patient up before running out to the station to call the MD:)GOOD LUCK..

Hi fellow nurses I think easy spirit clogs rule!!! I tried a pair and thats the only type of clog i wear.. They are soooooooo comfortable.. I can have a rough night and I go home and my feet don't ache!!!
